AFHTO's mission is to support the implementation and growth of FHTs by promoting best practices, sharing learning, and advocating on behalf of all FHTs. As of August 2009, 150 FHTs have been established and over 78 are members of AFHTO. |

On October 6, 2010, AFHTO gives you the opportunity to celebrate, share, and learn innovative best practices in all areas of FHT operations, including clinical practice, administration, and governance.
Ontario’s Family Health Teams are a unique innovation in primary care, and whether you have been a FHT for 5 years or 5 months, you can benefit from sharing your experiences and expertise with those facing similar challenges.
AFTHO Conference 2010 will offer a "marketplace" of 44 workshop/sessions featuring best practices, innovations, and ideas that you can "take home" and implement in your family health team. These sessions will be presented by AFHTO physicians, nurses, other interprofessional healthcare providers, executive directors, administrators, and board members as well as some invited experts. The small group sessions will give you a chance to dialogue with the presenters and to share ideas with one another.

By proactively addressing risks and opportunities, health care organizations protect and create value for their stakeholders – including patients, health care workers, government and the overall community.

All-Leaders Retreat Summary Report
Dear FHT Leader
As evidenced by the intense engagement, the positive feedback, and the consensus around next steps, the November 4th Leaders Retreat clearly fulfilled a common need. It provided the impetus and set the direction for the Family Health Teams to act collaboratively to fulfill their mandate and to secure their continuing support by the Ministry of Health and Long-Term Care. The FHT leaders expressed strong endorsement for AFHTO as a network, a resource center, a quality guide, and an advocate.
As promised, we are providing a summary of the discussions and recommendations from the All Leaders Retreat. For those who participated, we trust it faithfully represents your thoughts and opinions. For those who were unable to attend, we hope that it will give you a sense of how issues were identified and developed and the rationale for the recommendations that emerged. The goal is for all of us to commit to and fully participate in the next steps.
The Board has requested a meeting with Minister Deborah Matthews and will be providing a briefing to her chief policy advisor near the beginning of 2010. This will be based on the issues and feedback from the survey we conducted of all FHTs in the summer of 2009 and on the content of the All Leader’s Retreat
The Board will be holding a strategic planning session in mid-January. We will be developing a budget that will allow us to address the priorities you expressed in the Survey, the All Leaders Retreat and those arising from our strategic planning. We anticipate the need to reconsider our dues structure and will be advocating with the Ministry for the formalized approval of the inclusion of AFHTO dues in the FHT budgets. Obviously, the Board will consult with members with respect to a proposed fee structure and all other policy issues.

FHTs Gathering Momentum
Quick facts:

150 teams are currently providing care to more than 1.9 Million Ontarians of which 270,000 are new unattached patients.
- MOHLTC has announced the implementation of 50 new FHTs. New Family Health Teams will support other ministry initiatives including: reducing the number of unattached patients and improving chronic disease prevention and management with a focus on diabetes to support the Ontario Diabetes Strategy, and integrated cancer screening. New teams should be prepared to participate in implementing the Ontario Diabetes Strategy through approaches such as the development of diabetes programs and services that would be delivered through a Registered Nurse and Registered Dietitian or Health Educator.
- In June 2009, applications were solicited for 19 Wave 4 FHTs to be announced in fall 2009.
- The ministry expects to have a Wave 5 call for applications in the spring of 2010. Those interested in being placed on the expression of interest list for Wave 5, or wishing to ask other questions regarding the 50 new FHT platform commitments are asked to send an email to: fht.inquiries.moh@ontario.ca.
