The Association of Family Health Teams of Ontario

The Association of Family Health Teams of Ontario (AFHTO) is a not-for-profit, grass roots organization. AFHTO's goal is to promote and organize on behalf of all Family Health Teams (FHTs) in Ontario.

As of February 2007, 150 FHTs have been announced, 88 are operational and 55 have joined AFHTO.

AFHTO Seeks Board and Committee Members

AFHTO is actively seeking Board and Committee Members - you must be a member of a FHT to apply. Please submit your application to AFHTO.

“To support the implementation and growth of Family Health Teams across Ontario through: advocacy and promotion; undertaking of research in respect of primary care; disseminating best practices; sharing lessons learned; and providing relevant products and services.”

FHTs Gathering Momentum

Quick facts:

  • 150 teams, in various stages of implementation/operation, are currently providing care to more than 1.5 Million Ontarians of which 116,000 are new unattached patients.
  • 50 teams renovating space and 70 new physicians recruited totaling 1,355 involved physicians working in FHTs or almost 1 in 5 Ontario Family Physicians.
  • Currently, 88 teams have commenced operation and hired more than 423 allied health professionals including 99 Nurse Practitioners, 150 Registered Nurses, 36 Dieticians, and 49 Mental Health Workers.
  • 132 Business and Operational Plans submitted - 126 approved. Over the next few months these numbers will grow considerably as more teams become operational.
  • FHTs are currently developing and implementing Chronic Disease Management programs (such as Asthma, Diabetes, COPD, Osteoporosis) and Early Health Promotion and Disease Prevention.
