Outcomes:

The AFHTO conference will provide Family Health Teams allied healthcare professionals, administrators as well as Executive Directors and lead physicians with a sense of excitement and confidence about the future role of Family Health Teams. 

AFHTO has regularly scheduled meetings with the implementation team at the MOHLTC to assist FHT’s with relevant issues. We look forward to your input to enhance our collective understanding of implementation issues. We would encourage you to seek funding sources through your FHT budget and QMC initiative when and where possible. The FHT’s are at various stages of implementation and over 16 have been funded and publicly announced to implement Chronic Disease Management Approaches of the diabetic populations. 

This model is clearly in transition. Those leading primary care renewal have faced significant challenges and opportunities with health system change in Ontario.  Many are delighted at the new collaboration and assistance that we have experienced, sharing complex continuing care in the community. At this time, it is imperative to share the breadth of experience and debate the merits of successful approaches in quality improvement and population management in the area of chronic disease management arena. This conference will encourage frank and open discussions with FHT leadership to problem solve challenges that face us all.

Deliverables:

  • Frank examples of choices and free ranging discussion about the merits of Quality Improvement that has international perspective (US and UK) – what worked, what didn’t.
  • An understanding of what data registries, advanced access and pay for performance issues as they affect office management of diabetes patients for QI teams, complementing QMC Initiatives.
  • Provide examples of demonstrable success in key leadership challenge areas for FHT’s, such as communication, team building, visioning, collaboration (both internally and with external partners), and technology.
  • Facilitators will guide attendees through resolving common questions about policy, procedure, and other human resource questions.
  •  FHT leaders will be introduced to models for CDPM, tested and proven in the US setting and practice management that complement FHT resources.
  •  In keeping with AFHTO’s core mission, opportunities will be provided for FHT leaders to network and share their successes, skills, and initiatives.

Attendees will be provided with information about AFHTO’s associated products and services, the development of AFHTO (particularly board retreat and business plan), and opportunities to participate in AFHTO
